Enhance error messages for debugging

Enhance error messages to contain actual rule that was fired that caused error (Decision table name and row/column if possible).

Debugging errors that occur during rule execution is difficult and time consuming. Messages returned from execution and included in logs are only providing enough information to get to the rule package in which the error occurred. In many cases there are multiple decision tables in a rule package and many columns/rows in each table.

Example of error received indicating the null pointer occurred in Package 2.
